Accepting referrals is at the heart of what we do. We're dedicated to providing vital mental health and substance abuse treatment services, understanding the profound impact these challenges can have on individuals and their loved ones. Our commitment is to offer comprehensive, personalized support, welcoming referrals from healthcare providers, community organizations, and concerned individuals. General Referrals
We welcome referrals for any program or service at any time. To refer someone, simplydownload and complete our general referral form, then email it to referrals@slvbhg.org. Ourreferral specialist will contact the referred client within two business days to help them enroll in our services.
Judicial Referrals
Judicial referrals from all court systems throughout the San Luis Valley are accepted. Court officials who wish to refer a client to our services can download the judicial referral form and send it to judicialreferrals@slvbhg.org viaemail. Our referral specialist will contact the referred client within two business days to help them enroll in our services.
School-Based Services Referrals
We accept school-based referrals from all educational institutions within the San Luis Valley. School officials wishing to refer a student to our services can download theschool referral form and email it to schoolservices@slvbhg.org. A member of our School-Based team will contact the referredclient within two business days to help them enroll in our services.
