Start Your Care: Our Intake Process

We offer two convenient ways to complete the enrollment process: by phone or in person (walk-in).

Phone Enrollment

Typically takes 20–30 minutes, depending on client needs

Call 719-589-3671 and select Option 3 for Enrollment.

Connect with an enrollment clinician who will open your chart and assess your needs.

Review necessary forms and determine the best way to complete and sign them.

Schedule a comprehensive assessment with your assigned provider.

Walk-In Enrollment

Typically takes 30 minutes to 1 hour, depending on client needsand wait time

Fill out intake packet ahead of time or upon arrival.

Let front desk know you'd like to complete a walk-in enrollment.

Front desk opens chart and notifies enrollment clinician.

Meet privately with clinician to review paperwork and schedule assessment.

Client Advocacy

Client advocacy aims to prevent rights violations, advocate for the provision of quality care, and review, investigate and resolve rights complaints or issues.

Informs consumers of mental health services' rights
Helps clients speak for themselves or speaks on their behalf
Assists with complaints about rights and services

Client Handbook

Our Client Handbook is designed to help you understand your rights, responsibilities, and the services we provide. It includes important information about our policies, procedures, and what you can expect as a client.
